Hospital Questionnaires

The quality of the care we provide, the number of people we serve, and other metrics are essential to helping you make an educated choice regarding care.

These documents are publicly available reports that we share with the Georgia Department of Community Health (DCH), the Centers for Medicare and Medicaid (CMS) and others to provide greater insight into our hospital administration and operations.

We’ve placed these documents on our own site to make them more accessible to visitors and to give you the information you need to make the most informed choice possible concerning your care and the care for those you love.

Tanner has no imaging center surveys to report.
