The Tanner Medical Group Bridge Clinic is a transitional care facility designed to “bridge”the gap and support patients during the critical period between hospital discharge and a follow-up visit with a primary care provider (PCP).

Focusing on timely, accessible care

For patients that do not have a PCP, our team is here to answer questions and address any unresolved health concerns you may have while you work to find a suitable PCP and schedule your first visit.

Our services include medication management, chronic disease monitoring, coordination of care with specialists and patient education.

By focusing on timely and accessible care, we are able to help our patients recover more effectively while fostering long-term connections with primary care resources.
